Through a year-long Albert Schweitzer Fellowship, Carlie and Merrick have collaborated with The Beacon and HOMES Clinic to implement a resource navigation program for those experiencing in Houston, TX. As resource navigators, they addressed clients' social determinants of health through needs assessments and individualized care plans and used the Unified Care Continuum Platform (UCCP) from the Patient Care Intervention Center (PCIC) for care coordination. Through bi-weekly in-person follow-up and virtual support, they seek to foster continuous engagement with clients and provide adaptive care. With this project, Carlie and Merrick hope to expand the reach of HOMES Clinic student volunteers, grow in their understanding of the available resources for this population, and help meet the needs of those experiencing homelessness.

We also continue to conduct research aimed at measuring service quality to drive quality improvements. Current projects include:
